CONSENT TO THE PROCESSING OF PERSONAL DATA, THE ASSIGNMENT OF EXCLUSIVE RIGHTS, AND THE USE OF MATERIALS FOR THE PURPOSES OF THE 15TH CONGRESS OF ONCOLOGISTS AND RADIOLOGISTS OF THE CIS AND EURASIA*
By checking the relevant box on the Event website ([https://adior-onco2026.com](https://adior-onco2026.com)) (the “Website”), I, acting freely, of my own will and in my own interests, and in accordance with Federal Law No. 152-FZ dated 27 July 2006 “On Personal Data” and Part Four of the Civil Code of the Russian Federation, hereby confirm the following and give the consents set out below.
1. Personal Data Controller
The personal data controller responsible for processing my personal data is Agency for Support of Medical Initiatives LLC (APMI LLC, TIN 7724319482) (the “Controller”), acting as the organiser of the 15th Congress of Oncologists and Radiologists of the CIS and Eurasia (the “Event”).
2. Purposes of Personal Data Processing and Categories of Personal Data Processed
I hereby consent to the Controller, as well as to third parties engaged by the Controller for the purposes set out below (including, without limitation, technical contractors, Event sponsors, and other persons involved in the organisation and conduct of the Event), processing the following categories of my personal data:
• Surname, first name, patronymic;
• Job title, employer (full legal name of the organisation), academic degree and academic title;
• City and country of residence;
• Email address and contact telephone number;
• Image (including photographs and video footage) and voice (audio recordings) recorded during my presentation and participation in the Event;
• Quotations from and excerpts of my speech included in the presentation materials.
The above personal data shall be processed solely for the following purposes:
• organisation and conduct of the Event in a hybrid format, including in-person participation and online streaming;
• live streaming and recording of the Event using the Zoom and Facecast platforms;
• provision of technical support for the streaming;
• storage, record-keeping, and provision of access to video recordings of my presentation for Event participants by making such recordings available in participants’ personal accounts on the ADIOR Academy website ([https://lk.adior-academy.ru](https://lk.adior-academy.ru));
• preparation of reporting materials for Event partners and sponsors.
3. Cross-Border Transfer of Personal Data
I have been informed and agree that, due to the use of international online platforms (Zoom and Facecast) for the conduct of the Event, as well as the participation of foreign persons (speakers, delegates and viewers) in the Event, my personal data may be transferred to the territory of foreign states. Such states include, in particular, the CIS member states, as well as other countries in which the servers and other technical infrastructure of the platforms used are located. The Controller shall take measures to ensure that data processing is carried out using servers located in the territory of the Russian Federation to the extent technically feasible.
4. Assignment of Exclusive Rights and Use of Materials
In accordance with Articles 1229 and 1270 of the Civil Code of the Russian Federation, I hereby assign to the Controller the exclusive rights in and to the video materials, audio recordings, photographs, texts and graphics prepared by me and/or with my participation and forming part of my presentation and speech (the “Materials”), including, without limitation, the right to use the Materials in the following ways:
• reproduction of the Materials, i.e. making copies of them in any tangible form, including fixation in a computer’s memory;
• broadcasting of the Materials by air and by cable, including rebroadcasting;
• making the Materials available to the public in such a way that any person may access them from any place and at any time of their own choosing (including by making them available on information and telecommunications networks, including in the personal account at [https://lk.adior-academy.ru](https://lk.adior-academy.ru)).
The exclusive rights in the Materials are assigned, and the Materials may thereafter be used by the Controller, on a royalty-free basis, unless otherwise provided for in a separate agreement entered into directly by me with third parties.
5. Transfer of Materials to Sponsors
I agree to the transfer of the Materials (or parts thereof) to the Event sponsors for the purpose of preparing internal and public reports on the outcomes of the Event. I understand that any other use of my Materials by the sponsors beyond the stated purpose (including, in particular, use for advertising or marketing purposes) shall be permitted only on the basis of a separate agreement entered into between me and the relevant sponsor. The Controller shall not be liable for any actions of sponsors exceeding the scope of the authority granted under this Consent.
6. Term of the Consents and Data Retention
Personal data shall be processed for the period necessary to achieve the purposes set out in Clause 2 of this Consent, including ensuring participants’ access to the archive of recordings in their personal accounts. After the Event has ended and once the above purposes have been fulfilled or no longer require processing, the personal data shall be destroyed, unless otherwise required by the legislation of the Russian Federation.
The exclusive rights in the Materials are assigned for the full term of copyright protection (in accordance with the Civil Code of the Russian Federation).
7. Procedure for Withdrawal of Consent
I have been informed that I have the right to withdraw this Consent to the processing of personal data by sending written notice to the Controller’s legal address or an email to: [info@medbymed.ru](mailto:info@medbymed.ru). Upon receipt of such notice, the Controller undertakes to cease processing my personal data and to remove from access in the personal account any video recordings featuring my participation within no more than 30 (thirty) working days, unless otherwise required for compliance with the legislation of the Russian Federation. Withdrawal of consent shall not result in the cessation of any data processing required for compliance with legal requirements and shall not apply to data already transferred to third parties prior to such withdrawal.
8. Speaker’s Representations and Warranties
I hereby represent and warrant that I am the author of, or the lawful holder of the exclusive rights in, all Materials used in my presentation, and that the use of such Materials by the Controller in accordance with the terms of this Consent does not infringe the rights of any third parties.
In the event that any claims or lawsuits are brought against the Controller by third parties in connection with the infringement of their exclusive rights resulting from the use of the Materials, I undertake to settle such claims independently and at my own expense, and to indemnify the Controller for any losses incurred in connection with such claims.
9. Acknowledgement of Review of the Controller’s Documents
I confirm that, prior to giving this Consent, I reviewed the Personal Data Processing Policy of APMI LLC, publicly available at [https://lk.adior-academy.ru](https://lk.adior-academy.ru), and that the text of the Policy was accessible to me on the Website at the time of registration. My rights as a personal data subject have been explained to me and are clear to me.
